1. Associated financial responsibility (min. of $450) to be paid in full at time of service.
2. Except if related to anesthesia, less than 10% are able to recover to be discharged from hospital.
3. I understand will be updated with status reports as soon as possible once BLS is institute to make further decision(s).

By selecting NO, I understand and agree to the following:
1. Team COVE will contact me to review options and reconsider approving BLS, though due to the time sensitive nature of this, my pet will likely pass away during this time.
